1. Liability Waiver and Release By signing below, I, the undersigned parent/legal guardian of the above-named child, understand and agree to the following: Participation in the Everett Royals Youth Clinic involves physical activities that may carry certain risks, including but not limited to, minor injuries, falls, or accidents. I voluntarily assume all risks associated with my child’s participation in this clinic. I release and discharge Everett Royals, its coaches, staff, volunteers, and sponsors from any and all claims, liabilities, or demands arising out of my child’s participation, except in cases of gross negligence or intentional misconduct. I confirm that my child is physically able to participate in the activities and has no medical conditions that would prevent participation. 2. Medical Authorization In the event of a medical emergency, I authorize the Everett Royals staff or representatives to obtain medical treatment for my child if I or the emergency contact cannot be reached. I understand that I am responsible for any costs incurred for such medical treatment. 3. Social Media and Media Release I grant permission to the Everett Royals to: Take photographs, videos, or other media of my child during the clinic. Use my child’s image, likeness, voice, or name in promotional materials, social media posts, and other marketing efforts related to the Everett Royals. I understand that these materials may be used publicly and that I will not receive any compensation for their use. I waive any rights to inspect or approve the finished product, including written or electronic copy, wherein my child’s likeness appears. 4. COVID-19 Acknowledgment (if applicable) I acknowledge that participation in group activities carries a risk of exposure to communicable diseases, including COVID-19. I agree to comply with any health and safety guidelines set forth by the Everett Royals during the clinic. 5. Agreement to Terms By signing below, I confirm that I have read, understood, and agree to the terms outlined in this waiver and release form. I certify that I am the parent/legal guardian of the participant named above and have the authority to sign this document.
